This form should be used to submit a course substitution for graduate-level Georgia State coursework. You must list the course(s) you have taken and either the specific course it is substituting, or the area on your Degree Works academic evaluation that the course should fall into. The request will be reviewed by the Office of Academic Assistance – Graduate Programs, and you will receive a follow-up notification.

This form is only for courses taken at Georgia State University. For courses taken at other institutions, please use the transfer credit form.
